No Bleeding After the Abortion Pill: What It Means

First: bleeding can take time to start

After you take misoprostol, cramping and bleeding usually begin within a few hours — but it can take up to 24 hours to start. If it's only been a few hours, the process may simply not have gotten going yet. Staying warm, resting, and giving it time is reasonable in that window.

When "no bleeding" is actually a concern

Here's the part that matters most: no bleeding is not a sign that everything quietly resolved. For most people, a medication abortion involves noticeable bleeding and clots. If you've taken the full medication and still have no bleeding at all after about 24 hours, that usually means it hasn't worked — the pregnancy may be continuing — rather than that it's already done.

So if you're past that window with nothing happening, treat it as a reason to check in, not a reason to assume success.

What to do if you're not bleeding

  • If it's been less than 24 hours since misoprostol: bleeding may still begin. Give it time, and keep an eye on how you feel.
  • If it's been more than 24 hours with no bleeding: contact your provider or the service that sent your medication to talk through next steps. Don't assume it worked.
  • A follow-up pregnancy test can help, but timing matters — pregnancy hormones stay detectable for weeks afterward — so your provider can tell you when and how to test.

Warning signs that need urgent care

Call 911 or go to an emergency room if you have:

  • Severe pain on one side of your lower abdomen
  • Pain in your shoulder, dizziness, or fainting

These can be signs of an ectopic pregnancy — a medical emergency that does not cause typical abortion bleeding and needs immediate treatment.
